Three separate investigations have been launched into the death of a paramedic in Co Cavan yesterday including one by the Health and Safety Authority.
Reports suggest the door of the ambulance opened during a patient transfer from Cavan to Dublin near Stradone, Thursday, 3 June and the 43-year-old man fell out.
